Requesting for information for an external specialist
If your child is seeing an external provider (such as a paediatrician, psychologist, speech-language pathologist, or occupational therapist), Banksia Beach State School can provide helpful information about their learning, progress, and support needs.
To request this, please download and complete the External specialist request form here and return it to the school office or email it to ExternalRequests@banksiabeachss.eq.edu.au. External providers can also email the request directly to ExternalRequests@banksiabeachss.eq.edu.au. Printed copies of this document are also available at the front office.
We will then be in touch to arrange written consent before any information is shared.
